SandRiCare Services

SandRiCare services that are provided to our community include: 

Coordination of Supports:

• Understand your plan,
• Connect with Supports and Services
• Design Support Approaches
• Establish Supports,
• Crisis planning, prevention and mitigation and Action plan
• Report to NDIA

Community Nursing Services:

• Medication Management including subcutaneous injections
• Diabetes and other chronic disease management
• PEG/PEJ care and feeding
• ileostomy/colostomy care
• Wound care/dressings, and pressure injury prevention and management.
• Continence assessments and support; Indwelling and Suprapubic catheter care
• Post hospitalisation care and support
• Safety/Risk assessment and management.

Household Tasks:

• Domestic cleaning and laundry

Social and Community Participation:

• Shopping trips, sporting events/matches, and cinema visits
• Join and attend social/community groups for fitness and/or recreational activities

Assistance with Self Care Activities:

• Showering, dressing and grooming
• Mealtime support

Short Term Accommodation:

• Respite Care with round the clock clinical nursing support on call.
